Dear Church Member,
This is a brief survey asking for you to let us know about your interests and talents, which you would like to dedicate to God's ministry through our church. Please answer questions below, including your name, address, phone number and e-mail address, and then click on "Submit." This information will be protected in every way possible, and will only be used to improve the way we carry out the mission of Jesus Christ in the church.
